This is a direct care position that supports adults in the Augusta community who suffer with severe and persistent mental illness.   • Provide case management, support and community integration services that involves the identification, assessment, planning, linking, monitoring and evaluation of services and supports needed by clients.
  • Develop Individualized Support Plans in conjunction with clients and collaboratively update and revise plan as needed.

Sweetser’s Behavioral Health Specialist collaborates with primary care practitioners in the practice where they are integrated to provide brief, solution-focused interventions for patients whose overall wellness can be enhanced by a coordination of their physical and behavioral healthcare.

Schedule:  Year-round, Monday through Friday 8am to 3:30pm

  • Actively participate in analyzing the results of behavioral health screening tools.
  • Demonstrate competence in quick and accurate clinical assessment of behavioral health conditions.
  • Demonstrate an ability to communicate key facts and concerns in both formal and informal opportunities.
  • Demonstrate an excellent working knowledge of brief, evidence-based treatment modalities.
  • Demonstrate an ability to manage the pace of primary care and working with an interdisciplinary team.
  • Demonstrate ability to implement clinical pathways and protocols for treatment of certain chronic and/or co-morbid conditions.

Qualifications:

  • Master’s degree in social work.
  • Independently licensed to practice as an LCSW in Maine.
  • Ability to utilize technology as a primary support tool in practice.
  • Demonstrated knowledge of the trend toward Patient Centered Medical Homes, collocation of behavioral and physical healthcare, and specific behavioral health related issues such as trauma, substance abuse and recovery-oriented care.

Sweetser is looking for a Master's level Home and Community Treatment Clinicians to serve families and children by providing appropriate clinical services in their homes with the support of a Behavioral Health Professional.

Locations: Brunswick, Sanford, Lewiston, Saco, Rockport

Schedule: Monday through Friday.  Weekends and Holidays Off

Reimbursement for clinical licensure, exams and renewals. Clinicians are provided laptops, cell phones and mileage reimbursement.

  • Assess individual and family situations.  Understand and implement a systems perspective.
  • Provide a comprehensive level assessment to develop a treatment plan.
  • Identify goals for treatment.  Formulate and implement treatment goals and objectives.

Qualifications:

  • Master's degree in either Social Work, Counseling or Marriage and Family Therapy with a concentration in clinical practice from an accredited graduate school.

School-based Therapy Services provide individual, family and/or group therapy to children, adolescents and their families in the setting that is most comfortable for the unique needs of each family, which could include their child’s school, their home or another community setting. There are currently Sweetser community-based therapists working in more than 80 public schools throughout Maine.

As a School-based clinician, you will work full time providing comprehensive assessments, diagnosis, treatment planning, and psychotherapy to students and their families within the school system. You will also act as a liaison between program and family, participate in program development, and maintain clinical records.

Education Requirements:
